Disconnect a payment provider

Until now a connection could only be updated, not removed, and keys you once saved stayed with us. Each connection page now has a Disconnect button. It deletes the saved keys and the connection itself, so the page starts empty again. Product mappings, buy buttons and pricing tables stay and work again once you reconnect, and members already in Ghost keep their access. The confirmation reminds you to remove the webhook and the API key at the provider as well.
